We need your baby picture!!

We suggest a wallet size professional photo with a somewhat plain background if you have one available.

 If not, you can submit a 4 x 6 photo that you took, keeping in mind that it should be fairly close up (no distant full-body shots as they will not show off your child's face that well). Your picture does not have to fit in the box to the left ... it will be cropped and/or zoomed in or out to make it fit appropriately into the provided space.

** Please write your child's name and teacher on the back of the photo, making sure that you don't press too hard on the photo. You will most likely NOT get your picture back, so please DO NOT SEND YOUR ONLY COPY. **

Put your photo in an envelope labeled "PTA Mailbox, 5th grade baby picture" with your child's name and teacher as well. Please submit your photo before December 16.
